Jackson Hospitals’ Community Health Needs Assessment Survey Now Available
MONTGOMERY, Ala., May 15, 2025 – On May 1, Jackson Hospital & Clinic launched their 2025 Community Health Needs Assessment (CHNA).
Conducted every three years, the CHNA a systematic process of gathering and analyzing data to identify the community's most pressing health needs and to guide the hospital’s future program offerings. A key component of the CHNA is a community-wide survey available online at
https://forms.office.com/r/80pkUUa2db, available from May 1 until June 30, and Montgomery County residents age 18 and older are encouraged to participate. The process also includes one-on-one interviews with community leaders.
“Everyone’s input is welcome; by understanding our community's unique health concerns and experiences, we can better tailor programs and services to address pressing issues,” said Gina Anderson, Jackson Hospital COO & CNO. “The top needs identified in the 2022 CHNA were Diabetes, Cardiovascular Disease, Obesity, Health Education, Financial Barriers, Access to Care, and Smoking Cessation. It will be informative to see if they are the same this year.”
The hospital’s goal is to have 500 completed surveys by June 30. It is completely anonymous and only takes about 10 minutes to complete.
The company performing the assessment is VMG Health. The survey covers access to care, community health strengths and barriers, social determinants of health, and more. All responses will be compiled and carefully analyzed to inform the development of a CHA Report, which will be available to the public in early 2026. This report will serve as a roadmap for prioritizing health initiatives and allocating resources effectively through a health improvement plan.
